(a) For Applicants requesting assistance for themselves (Section A: Q1a) or as Caregivers/Third Parties for older persons or persons with disability (Section A: Q1b, c)

1. Are you (/the person you are applying for) aged 65 years or above?

Yes

Skip to Q3 if answer is No

2. Do you (/the person you are applying for) receive any form of superannuation or pension (Ex-Serviceman funds, Government or FNPF Pension, other)?

Yes

3. Do you (/the person you are applying for) have a disability?

Yes, permanent disability

Yes, mild or moderate disability

Note: With respect to disability, the applicant could be any age (i.e. answer to Q1 can be yes or no)

4. Are you (/the person you are applying for) pregnant and receiving antenatal care from a MHMS rural health facility or an urban hospital?

Yes

(b) For Applicants requesting assistance for their families (Section A: Q1d) or as Caregivers for children (Section A: Q1e)

1. Is your family (/ the person you are applying for) facing hardship?

Yes

2. Are you / the person you are applying for caring for a vulnerable child/children aged less than 18 years?

Yes, child is under the care of single parent or deserted spouse with no child maintenance

Yes, child is in the care of Foster parents/guardians

Yes, child is in a family with death or incarceration of breadwinner

Note: The Caregiver can also be eligible for SPS/DAS/TAS depending on their responses to Section C (a)
